Our Continuity-Of-Care Team provides the following services:

• Patient Assessment - Eligibility requirements for insurance acceptance and reporting to primary physician on patient performance for respiratory patients
• Training - In hospital training, multi-media, audio visual training materials and problem solving
• Pharmacy Supply - Aseptic compounding of solutions, identifying and manging complications and prescription drug counseling
• Medical Equipment and Supplies, Respiratory Therapy and Orthopedic Appliances. Home evaluation of equipment needs, initial equipment set-up and delivery, regular equipment maintenance, service and repair specialists, patient fitting/customization.

Our patients will receive:

• Routine clinician follow-up for respiratory patients 
• 24 hour emergency call
• Individualized delivery schedule
• Reinforcement training
• Personalized reimbursement services including timely handling of all insurance claims, Direct third party billing (drug cards accepted) Insurance negotiations for extended benefit claims.
